Does Orajel Make Canker Sores Worse?

‍

Orajel numbs canker sore pain and neither heals nor worsens a typical sore, which clears on its own in 1 to 2 weeks. We hear the opposite worry from patients at our Woburn office, and there is usually a real reason behind it. A gel can sting, an ingredient can irritate, and a sore that outlasts 7 days of numbing needs more than another tube.

Does Orajel make canker sores worse?

Orajel does not make a typical canker sore worse. The gel dulls pain for a short stretch, and the ulcer underneath keeps healing on its own schedule. Canker sores go away on their own in 1 to 2 weeks, and treatment is not usually required, according to the National Institute of Dental and Craniofacial Research (NIDCR).

When a sore feels worse after Orajel, one of four things is usually going on.

What you noticeWhat is going onWhat to do
A sharp sting from the rinseThe original Orajel rinse contains 4.1% alcoholSwitch to the alcohol-free rinse, or stop
Pain comes back when the numbness fadesBenzocaine relief is temporary, and the sore has not changedExpected
Redness, swelling, or a rashA possible reaction to a "caine" anestheticStop and call a dentist or doctor
Sore unchanged after 7 days of useThe label's time limit has been reachedHave the sore examined

The first two rows are common and harmless. The last two mean stop.

What does Orajel do to a canker sore?

Orajel numbs a canker sore so eating and talking hurt less. Orajel 3X Medicated For All Mouth Sores Gel contains 20% benzocaine, 0.1% menthol, and 0.15% zinc chloride. Benzocaine is the oral pain reliever. Menthol is a second pain reliever, and zinc chloride is an astringent.

The Drug Facts panel describes the gel's job as temporary relief of pain, and healing appears nowhere in that description.

Numbing changes how a canker sore feels. The repair work is done by your own tissue, on the same 1 to 2 week timeline, whether the sore is numb or not.

A gel also does nothing about why canker sores form in the first place, which matters more for anyone who gets them several times a year.

Why does Orajel burn on a canker sore?

The likeliest reason Orajel burns on a canker sore is alcohol in the rinse. Orajel Antiseptic Rinse For All Mouth Sores pairs 1.5% hydrogen peroxide and 0.1% menthol with 4.1% alcohol by volume. NIDCR tells people with canker sores to skip mouthwashes that contain alcohol, since alcohol could irritate the sores.

Orajel also sells an alcohol-free version of the same rinse. Anyone who felt the original rinse burn has a simple first move in switching bottles.

The 3X gel is a different product, and its label lists no ethyl alcohol. Burning that lingers after the gel, or a sore that looks redder each day, matches a warning printed on the tube. The gel's label says to stop use and ask a dentist or doctor if irritation, pain, or redness persists or worsens.

When is Orajel the wrong choice for a canker sore?

Orajel is the wrong choice for anyone who has reacted to a local anesthetic, for children under 2, and for a sore that has already had a week of it.

The gel's allergy alert names procaine, butacaine, benzocaine, and other "caine" anesthetics. A past reaction to any of them, at a dental visit or anywhere else, is reason to leave the tube on the shelf.

Age matters too. The FDA has warned that over-the-counter benzocaine products should not be used on children younger than 2 years because of methemoglobinemia, a serious blood disorder. For children between 2 and 12, the Orajel label says to ask a doctor first.

Recurring canker sores in kids and teens deserve a dental visit more than a numbing gel, since an injury such as a bitten cheek is a known trigger.

Red flags: stop using Orajel and get medical help right away if any of these appear after use.

  • Pale, gray, or blue colored skin
  • Shortness of breath or a rapid heart rate
  • Headache, dizziness, or lightheadedness
  • Fatigue or lack of energy

The gel's label lists those symptoms as reasons to seek immediate medical attention.

How often can you use Orajel on a canker sore?

The Orajel 3X gel label allows use on the affected area up to 4 times a day, for no more than 7 days unless a dentist or doctor directs otherwise. The label is the limit.

More gel does not mean more healing, because the gel was never doing the healing.

Seven days is also a useful checkpoint. A canker sore that still needs numbing after a week has outlasted what the product is for, and the label says to stop and ask a dentist or doctor if symptoms have not improved in 7 days.

What makes a canker sore worse?

Friction, acid, and alcohol are the things that make a canker sore worse.

Numbing gel works like earplugs at a construction site. The noise stops reaching you, and the building takes exactly as long to finish. Meanwhile, whatever keeps knocking into the site is still there.

  • Hot, acidic, or spicy foods, which NIDCR says cause pain in existing ulcers and may trigger new ones
  • Mouthwash that contains alcohol
  • A sharp or rough surface that rubs the same spot, such as a chipped tooth or a worn filling
  • An accidental cheek or lip bite, which NIDCR lists among canker sore triggers

The rubbing kind is the one we can fix in the chair. Dr. Guldalian checks whether a tooth edge or filling margin lines up with the sore, and can have a rough filling edge smoothed or replaced during a routine visit.

A canker sore that keeps returning to the same spot usually has something physical behind it.

Dr. Guldalian can check your restorations, appliance fit, and brushing habits in a single visit.

What helps a canker sore heal?

Time heals a canker sore, and a few gentle habits make the wait easier.

Recommended:

  • Rinse with 1/2 teaspoon of salt in 8 ounces of warm water several times a day
  • Let ice chips dissolve slowly over the sore
  • Brush gently with a soft toothbrush and a toothpaste without a foaming agent
  • Skip rough, acidic, and spicy foods until the sore closes

Orajel fits into that routine as the comfort option. Used within the label, the gel makes meals tolerable while the sore closes.

Our separate guide to ways to shorten how long a canker sore lasts covers the prescription options a dentist can add when home care falls short.

When should a dentist look at a canker sore?

A dentist should look at a canker sore that has lasted longer than 2 weeks, since that is past the normal healing window. Other reasons to book a visit:

  • 7 days of Orajel with no improvement
  • Canker sores that keep coming back
  • Pain severe enough to make eating or drinking difficult
  • Swelling, a rash, or a fever alongside the sore

An appointment at our Woburn office is a short one. Dr. Guldalian looks at the sore, checks the teeth and restorations around it for sharp edges, and asks what you have been putting on it. Bring the tube.

Dr. Guldalian trained in prosthodontics at UCSF, so the way fillings, crowns, and appliances meet soft tissue is familiar ground for her. Much of that also gets checked during a routine exam.

TL;DR: does Orajel make canker sores worse?

  • Orajel numbs canker sore pain and does not heal or worsen a typical sore.
  • Canker sores clear on their own in 1 to 2 weeks.
  • The original Orajel rinse contains 4.1% alcohol, and alcohol could irritate a canker sore. An alcohol-free rinse is sold.
  • The Orajel 3X gel label allows up to 4 uses a day for no more than 7 days.
  • Benzocaine products should not be used on children younger than 2 years.
  • A canker sore that lasts longer than 2 weeks should be examined.

A canker sore past the 2-week mark, or past 7 days of gel, deserves a short exam.

Dr. Guldalian will find out whether something is rubbing, and will tell you plainly if the answer is more time.

Orajel and canker sore FAQs

Does Orajel heal canker sores faster?

Orajel does not heal canker sores faster. The gel's label describes temporary relief of pain, and its main active ingredient, benzocaine, is a numbing agent. Canker sores clear on their own in 1 to 2 weeks. Orajel makes eating and talking more comfortable during that stretch.

How long does a canker sore last if I use Orajel?

A canker sore treated with Orajel follows the usual timeline of 1 to 2 weeks. The label limits use to 7 days, so a sore that still hurts after a week of gel should be shown to a dentist or doctor. Any sore present longer than 2 weeks needs an exam.

Can children use Orajel on a canker sore?

Children younger than 2 should never be given Orajel or any other benzocaine product, according to the FDA, because of the risk of methemoglobinemia. For children ages 2 to 12, the label says to ask a doctor first and to supervise use. A child with frequent canker sores should see a dentist.

What is the difference between Orajel gel and Orajel rinse?

Orajel 3X gel is a 20% benzocaine numbing product applied to one sore. Orajel Antiseptic Rinse is a 1.5% hydrogen peroxide cleanser with menthol, swished around the whole mouth. The original rinse contains 4.1% alcohol, which could irritate a canker sore, and an alcohol-free version is available.

Can I use Orajel on a cold sore too?

The Orajel 3X gel label lists cold sores and fever blisters alongside canker sores for temporary pain relief. Cold sores are a separate condition. Fever blisters form outside the mouth around the lips, are caused by herpes simplex virus, and are contagious. A doctor can prescribe antiviral medicine when one is needed.
